Personally appeared before the undersigned officer, duly authorized to administer oaths, the affiant named below, who on oath states that:

1. Affiant is the landlord or the agent of the landlord of the premises described below, located in the county named above.

2. The tenant named herein is in possession of said premises and is indebted to affiant for rent in the amount stated, which remains due and unpaid after demand for possession was made pursuant to O.C.G.A. § 44-7-50.
